Volume, first. You're charged for what actually goes on the truck, not for how long the day takes.
After that it's access. A basement with a turn at the bottom of the stairs takes longer than a garage you can back up to. Both get quoted before anything is loaded, and the number holds unless you add to the pile.

Yes, and that's often the whole job. Pressed-wood furniture, older mattresses and appliances that don't run get turned down by charities. They go on our truck.
All of it, including the shed itself if you want it gone. Mention it in your request so it's in the price from the start.
Hazardous material. Paint, solvents, pool chemicals, propane tanks, oil, ammunition. Those need a household hazardous waste drop-off. The crew will point out anything in that category rather than quietly leaving it in the garage.
